The consent will be good for this school year. There will be 3 visits for Fluoride and 1 visit for Sealants.
Please fill out the online consent for YES or NO. All consents help the program receive their grants to help them provide these services. They will see all children that sign up and there is no fee for services.
If you do not know your child's teacher's name, just put the grade they will be in.
